Eligibility Requirements
- The nominee must be a legal permanent resident of the United States, residing permanently in Elk Ridge, Genola, Mapleton, Mona, Nephi, Payson, Salem, Santaquin, Spanish Fork, Springlake, and Springville.
- The nominee must own and reside in the detached, single-family home listed in the nomination form. The nominee must provide proof of ownership. Finalists must allow access to their home for Sponsors to assess the safety and feasibility of installation of the system. That assessment may be taken into consideration in selecting the final recipient.
- Mobile homes and manufactured/pre-fab homes are not eligible.
- The home must already have central forced air gas heating system with installed ductwork. The system does not have to be operational, however, the ductwork, gas and other attaching equipment must be installed.
- The nominee and the nominator must not be an employee or an immediate family member of any of the sponsors.
- The nominee and nominator agree to be bound by all terms and conditions.
- Finalists must sign a declaration of eligibility, a liability release, and a publicity release allowing sponsors to use their information and images for publicity purposes without compensation, and an agreement to accept the style or model of furnace based on the recommendation of Nebo Comfort, in the event they are selected before they will be eligible for final consideration.
- The nominator should only provide information in relation to the nominee if they have received permission from the nominee. Sponsors agree to hold that information until the nominee has signed required release. Sponsors are not liable for any information the nominator gives that they did not receive permission to give.
- When selected, the recipient agrees to make themselves available to sponsors between December 15, 2019 and January 1, 2019 to install the furnace.
- Nominee must demonstrate that replacing their existing furnace would constitute a financial hardship. Finalists may be required to provide documentation establishing financial need.
- Financial need is determined by the sponsors and may include:
- Hardship due to health-related expenses of the nominee or an immediate family member.
- Military service of the nominee or immediate family.
- The nominee serves in the community as a first responder, teacher, or similar occupation that has made a significant impact in the community.
- Eligible for a financial support program, including, but not limited to: food stamps, medicaid, or any low income energy assistance program.
